- Initial Screening Event : where vision and hearing testing is implemented
- Rescreen Event : where repeat vision and/or hearing test occurs for students who previously failed a test. (Scheduled10-14 days following Initial Screening Event)
- Threshold Testing : (audiograms) following hearing test failures at Rescreen Event
- Monitoring : Via Annual Threshold testing as long as deficit exists or student graduates
- Follow-up/Referral Work : Involving written and/or verbal communications with parents/guardians to share screening results and recommendations of Illinois Department of Public Health for further examination.
- Required Documentation and Reporting : (Illinois Department of Public Health Vision and Hearing Conservation Annual Reports are due June 30th each year)
